  1. Each form has its own inclusion code which has been generated automatically for you.

  2. 'Click To Get Code' from the above table, and a page will prompt out with an inclusion code. Then, copy and paste the code into your .php file, e.g. enquiry.php where you wish to display your Enquiry Form.
